Heather,

Yes many clients retain existing contents for two reason one is cost and another is shortage of Good SEO copywriters :) who can actually find out the competition of client keyword phrases and SEO writing with proper keyword density. Awareness on these can be brought though showing the keywords competition and your suggestions with client site keywords search results. Hope this can make client understand better about the importance of content update and they will have chance to think on further resource and investment

Excellent post. Another question relating to number seven that I would like to suggest is, "In order to comply with keyword density recommendations, would you be willing to accept a style of writing that is different from that which you are used to?" I've found that many clients are married to their existing content and have a difficult time accepting the nuances of SEO writing. They are sometimes unwilling to let go of their content at the cost of bettering their search engine performance.
